The Health Records Department provides transcription services, statistics for utilization management review, chart completion, medical/legal correspondence, coding and abstracting, ongoing quality assurance activities, research audits, microfilming, and control of patient records. Health Records is responsible for liaison with Shared Library Service in the Huron region and maintaining the hospital library. Transcription services are available to staff physicians and departments such as radiology and speech pathology. Pathology reports are transcribed at the Regional Laboratory located at the Stratford General hospital.

Health Records stores inpatient and day surgery records as well as nurses’ notes; outpatient and emergency records are stored in the Outpatient Department.

The department operates from 0800 to 1600 hours, Monday to Friday.

Health Records is working towards a fully integrated electronic Health record.
